Legal Service Plan Legal Security Package
Important -- The Legal Security Package benefit is effective with new enrollments as of Sept. 1, 2008, and renewals on or after Sept. 1, 2008.
- You must have purchased the Legal Service Plan on or after Sept. 1, 2008, or have renewed your existing Plan on or after Sept. 1, 2008 before these documents can be processed by the National Legal Office of Feldman, Kramer & Monaco, P.C. (or Glantz & Glantz, P.A. in Florida only).

Once you have purchased or renewed the Legal Service Plan on or after Sept. 1, 2008, you are entitled to one Legal Security Package per year of plan enrollment. The Package contains forms you must complete in order to receive a Health Care Proxy, Living Will, Durable Power of Attorney and Simple Will from the National Legal Office (or Florida State Legal Office).
All documents in the Legal Security Package must be prepared for the same Covered Individual (NYSUT member or his or her Covered Dependent).
Please refer to your Certificate of Legal Services Coverage for more information about the Legal Security Package. Certificates prior to Sept. 1, 2008 do not include this benefit.
