Legal Service Plan Legal Security Package
The Legal Security Package is one benefit of the Legal Service Plan.
- You must have purchased the Legal Service Plan to receive the Legal Security Package benefit.

Once you have purchased or renewed the Legal Service Plan, you are entitled to one Legal Security Package per year of plan enrollment. The Package contains forms you must complete in order to receive a Health Care Proxy, Living Will, Durable Power of Attorney and Simple Will from the National Legal Office (or Florida State Legal Office).
All documents in the Legal Security Package must be prepared for the same Covered Individual (NYSUT member or his or her Covered Dependent).
