Group Access Legal Service Plan Legal Security Package
Important -- The Legal Security Package benefit is effective with Group Access Legal Service Plan years that begin on or after Sept. 1, 2008.
- The Group Access Legal Service Plan is provided to you from your local association, benefit fund, or through contract negotiations with your employer. Be sure you are a participant in the Group Access Plan and not the Group Prepaid Plan.
- If a Group Access or Prepaid Legal Plan is not provided to you (from your local, benefit fund or employer) and you would like to purchase the voluntary Legal Service Plan, click here.
Participants in the Group Access Legal Service Plan with plan years beginning on or after Sept. 1, 2008 are entitled to one Legal Security Package per year of plan enrollment. The Package contains forms you must complete in order to receive a Health Care Proxy, Living Will, Durable Power of Attorney and Simple Will from the National Legal Office of Feldman, Kramer & Monaco, P.C. (or Glantz & Glantz, P.A. in Florida only).
All documents in the Legal Security Package must be prepared for the same Covered Individual (Group Plan participant or his or her Covered Dependent).
Please refer to your Certificate of Legal Services Coverage for more information about the Legal Security Package. Certificates issued prior to Sept. 1, 2008 do not include this benefit.
