What It IsThis insurance provides coverage 24 hours a day, 365 days a year against any type of accident in the course of business or pleasure that results in death or loss of a limb or other extremity. The plan also includes Worldwide Emergency Travel Services and an education benefit. Accidental Death & Dismemberment Insurance is underwritten by First Unum Life Insurance Company. The First Unum Life Insurance Company’s Accidental Death & Dismemberment Plan is a NYSUT Member Benefits Trust (Member Benefits)-endorsed program. Member Benefits has an expense reimbursement/endorsement arrangement of 5% of gross premiums, $.005 per $1,000 of benefits per month for individual coverage, and $.01 per $1,000 of benefits per month for family coverage for this program. All such payments to Member Benefits are used solely to defray the costs of administering its various programs and, where appropriate, to enhance them. Member Benefits acts as your advocate; please contact Member Benefits at (800) 626-8101 if you experience a problem with any endorsed program. Agency fee payers to NYSUT are eligible to participate in NYSUT Member Benefits Trust-endorsed programs.
EligibilityNYSUT members and agency fee payers may enroll. Members have the choice of individual or family coverage. Family coverage includes spouse or domestic partner who is living with the member, unmarried dependent children under age 19, or under age 25 if they are declared as legal dependents on your income tax forms and are enrolled as full-time students.
FeaturesAccidental Death & Dismemberment Insurance provides 100% of the elected amount of coverage in the event of accidental death. It provides reduced benefits in the event of dismemberment. You choose a coverage level of $25,000; $50,000 or $100,000. You choose individual or family coverage. Family coverage provides dependents with a benefit level that is a percentage of the primary insured's benefit level.
Worldwide Emergency Travels Services (WETS) provides worldwide medical and travel emergency services to the insured and his or her eligible dependents when they travel 100 miles or more from home or in a foreign country for business or pleasure. This feature is provided to your dependents even if you choose individual coverage for the AD&D policy. WETS refers insureds to qualified English-speaking physicians or medical facilities in other countries; secures hospital admission in other countries; returns seriously ill, injured or deceased insureds to the U.S.; provides lost luggage assistance, and many other services. Multi-lingual personnel, including physicians, staff this service 24 hours a day for consultation and assistance. WETS services are provided at no cost. However, the cost for any medical services received from a physician, hospital, pharmacy, etc., is your responsibility. Please note: WETS services are not provided for spouses or domestic partners traveling on company business. Note: If you receive Group Term Life Insurance and/or Group Long Term Disability Insurance coverage through your employer or local association's benefit fund, you may already have WETS coverage. Confirm this with your employer, local or NYSUT Member Benefits Trust. The AD&D plan includes an Education Benefit that provides 5% of an insured's AD&D benefit, up to $5,000 per year for up to 4 years, for a qualified child under age 25 who enrolls within 365 days of the accident causing your death or is already enrolled in an accredited post-secondary institution of higher learning on a full-time basis. The program has a 30-day free look period. If, after reviewing the Certificate of Coverage, you're not completely satisfied, return the Certificate within 30 days of receipt to Member Benefits for a full refund. If any of the Assist America services were used within the 30-day period, you will be billed on a fee-for-service basis for services used.
