Group Accidental Death & Dismemberment Insurance

What It Is

The NYSUT Member Benefits Trust-endorsed Group Accidental Death & Dismemberment Insurance Plan is only available for purchase by local associations, their benefit funds, and employers. The Group Accidental Death & Dismemberment Insurance Plan covers participants 24 hours a day, 365 days a year against any type of accident in the course of business or pleasure that results in death or loss of a limb or other extremity. The coverage provides 100% of the elected amount in the event of accidental death and reduced benefits in the event of dismemberment.

Enhancements to this program include Worldwide Emergency Travel Services and an Education Benefit.

The First Unum Life Insurance Company’s Group Accidental Death & Dismemberment Plan is a NYSUT Member Benefits Trust (Member Benefits)-endorsed program. Member Benefits has an expense reimbursement/endorsement arrangement of 5% of gross premiums for this program. All such payments to Member Benefits are used solely to defray the costs of administering its various programs and, where appropriate, to enhance them. Agency fee payers to NYSUT are eligible to participate in NYSUT Member Benefits Trust-endorsed programs.

Eligibility

The NYSUT Member Benefits Trust-endorsed Group Accidental Death & Dismemberment Insurance Plan is only available for purchase by local associations, their benefit funds, and employers.

All participants of your group may participate in the plan. Plan purchaser decides if Group Accidental Death and Dismemberment Insurance is provided to only in-service participants, or to in-service and retiree participants. Spouse/domestic partner and dependent child coverage can also be purchased by your group.

Features

• Minimum coverage amount is $10,000 per covered participant. Plan can be purchased in $10,000 increments up to a maximum coverage amount of $500,000 per covered in-service participant and $250,000 per covered retired participant.
• Plan provides 100% of the elected amount in the event of accidental death and provides reduced benefits in the event of dismemberment.
• Worldwide Emergency Travel Services (WETS)  is an enhancement to the Group Accidental Death & Dismemberment (AD&D) Plan. This is a worldwide medical emergency service available to all covered participants when they travel 100 miles or more from home or in a foreign country for business or pleasure. (WETS is also included with NYSUT Member Benefits Trust-endorsed Group Term Life and Group Long Term Disability Insurance Plans.)
• Education Benefit is another enhancement to the Group AD&D Plan. This benefit provides 5% of a participant’s AD&D benefit to $5,000 per year for up to 4 years for a qualified child who enrolls within 365 days of death or is already enrolled in an accredited post-secondary institution of higher learning on a full-time basis. This benefit is payable in addition to the face amount of coverage in the event of death as a result of injury.

Plan Options

Plan purchaser can select:

• Accidental Death & Dismemberment Benefit amount – available in increments of $10,000, with a minimum of $10,000 and a maximum of $500,000 for in-service participants and $250,000 for retired participants.
• Single coverage – the plan of benefits can be purchased for in-service participants, or in-service and retiree participants.
• Family coverage – the plan of benefits can be purchased to cover participants and their dependents.

Obtain A Quote

The cost for Group Accidental Death & Dismemberment is less than $.03/$1,000 of coverage per month for participant only (single) coverage, and less than $.04/$1,000 of coverage per month for dependent (family) coverage. As an example, the total cost for a 200-member local purchasing $20,000 of coverage for single coverage will equal $1,056 annually.

To obtain a quote, please provide amount of coverage desired, the number of participants if participant only (single) coverage is required, or if dependent (family) coverage is desired, then provide the number of single coverages and the number of family coverages.

Implement A Plan

Once the decision has been made to purchase the NYSUT Member Benefits Trust-endorsed Accidental Death & Dismemberment Insurance Plan on a group basis, here’s what needs to be done to implement the plan:

• Complete policy application.
• Provide contact name/address/phone number.
• For participant only coverage (single), provide the number of participants to be covered.
• For dependent coverage (family), provide the number of participants with single coverage and the number of participants with dependent (family) coverage.
• Certificates of coverage and other plan materials will be provided electronically to group contact for distribution.
