To report an address or name change, submit the address/name change in writing, and mail it to the address above or fax it to (518) 213-6413. Please include the NYSUT member's name, signature and Social Security number or NYSUT ID number and the old version of the information to be changed as well as the change itself. For privacy reasons, we cannot accept this information via e-mail. If you are a member of UFT, PSC/CUNY or UUP, you must contact them to report your address change; they will then transmit corrected information to NYSUT.
