MAP - Member Assistance Program
In efforts to expand communications with members, NYSUT Member Benefits Trust is introducing MAP, the Member Assistance Program. Just as maps provide direction, MAP team members will assist Member Benefits and vice versa. Through this e-mail-based program:
· You’ll be among the first to hear about new, endorsed programs and program updates.
· You’ll receive special offers.
· We’ll ask for your opinions on new programs under consideration.
When possible, we'll ask you to help test potential new programs and give us your feedback.
Your opinions matter! You can help us shape future programs and services exclusively available to you and your colleagues. Help make a difference!
We will not share your e-mail address with outside interests other than endorsed Member Benefits program providers, and only under controlled circumstances. We will oversee the frequency of e-mail communications – You have our word we will not bombard you with e-mails!

Eligibility
- Only NYSUT members and agency fee payers are eligible.
- VOID IN PUERTO RICO, THE VIRGIN ISLANDS AND WHERE PROHIBITED.
- Offer limited to legal residents of the United States who are 18 years or older.
- The following persons are not eligible to enter the Drawing: (a) employees, agents, consultants, officers and members of the board of trustees of NYSUT Member Benefits Trust ("Member Benefits"); (b) employees, officers and members of the board of directors of NYSUT; (c) employees and officers of any organizations directly involved with this promotion; and (d) immediate family members and persons living in the same household as the persons identified in 4(a) through 4(c) above.
Entry
1. NO PURCHASE OR PAYMENT OF ANY KIND NECESSARY TO ENTER OR WIN THIS DRAWING (the “Drawing”).
2. NYSUT members need not be present to win.
3. NYSUT members are not required to: (a) purchase goods or services, (b) pay any money, or (c) submit to a sales promotion, to be eligible for the Drawing.
4. NYSUT members may enter to win by registering on www.memberbenefits.nysut.org (the “Web site”).
5. Entries must include name, complete mailing address, e-mail address and name of local association to be valid.
6. Limit one (1) entry per member or agency fee payer.
7. Drawing opens at 12:00:00 a.m., ET, July 1, 2008, and ends 11:59:59 p.m., ET, September 30, 2008.
8. NYSUT Member Benefits Trust is not responsible for entries that are corrupted or otherwise not received correctly.

9. NYSUT Member Benefits Trust shall have no liability whatsoever for lost, late or misdirected entries.
10. All entries become the exclusive property of NYSUT Member Benefits Trust and will not be returned.
11. One (1) winner will be chosen from among all eligible entries by a random drawing in Latham, New York on or about October 2, 2008. The winner chosen will receive one (1) Digital Camera (Approximate Retail Value $100).
12. Odds of winning depend on number of eligible entries received.
13. Except as specifically set forth herein, all information provided by entrants will be subject to the NYSUT Member Benefits Trust Privacy Policy. The Privacy Policy may be viewed on the NYSUT Member Benefits Trust Web site: or click here, or may be obtained by writing to: NYSUT Member Benefits Trust, 800 Troy-Schenectady Road, Latham, New York 12110-2455. Such Privacy Policy is subject to change at any time, without notice, by NYSUT Member Benefits Trust.
14. By entering, each participant agrees: (a) to be bound by these Rules and the decisions of NYSUT Member Benefits Trust, which are final and binding in all respects; and (b) to release and hold harmless NYSUT Member Benefits Trust and its officers, employees, agents and directors from any and all liability for injuries, death, loss, tax liability, or damage of any kind arising from the participant's participation in this drawing or the acceptance, use or misuse of the prize.
15. The winner's name will be posted on the NYSUT Member Benefits Trust Web site on or about October 3, 2008.
16. The winner(s) will receive confirmation by e-mail (if registered through the Member Benefits Web site) within five (5) business days from date of the drawing.
17. Acceptance of a prize signifies winner’s consent that NYSUT Member Benefits Trust or its agents may, without further consideration or restriction use and/or publish the winner's proper name, city, state, likeness and prize information, for promotional purposes, whether online or in print or in any other media in connection with the Drawing, where legal. (Tennessee residents need not consent to, or sign, any publicity release).
18. The winner may be required to sign a notarized affidavit or a declaration of eligibility and a release of liability in favor of NYSUT Member Benefits Trust to receive the prize. If required, the affidavit or declaration must be returned within five (5) days of receipt by the winner. In the event of noncompliance with these requirements, or in the event that the affidavit or declaration, the prize or prize notification is returned as undeliverable, the prize will be forfeited and an alternate winner will be selected.
19. The prize is not transferable by the entrant and may be subject to certain restrictions. No cash redemption or prize substitution is permitted except at NYSUT Member Benefits Trust's sole discretion, and then only for a prize of equal or greater value.
20. All taxes, including without limitation, state, federal, and local sales, and income taxes, luxury taxes, and other expenses or costs associated with obtaining and using the prize are solely the responsibility of each winner. The winner agrees to indemnify, defend and hold harmless NYSUT Member Benefits Trust from and against any claims, damages or penalties of any taxing authority or other government agency or third party in respect of such taxes or expenses.
21. Prize, prize information, affidavits and statements of eligibility, if any, will be shipped to the address stated in the winner’s entry.
General Conditions
22. This Drawing is sponsored and promoted by NYSUT Member Benefits Trust, 800 Troy-Schenectady Road, Latham, New York 12110.
23. The Drawing is governed by the laws of the State of New York, and all claims relating to the Drawing must be brought in the state or federal courts located in Albany County.
24. Requests for the name of the winner may be sent to the following address: NYSUT Member Benefits Trust, ATTN: Patrick Regan, 800 Troy-Schenectady Road, Latham, New York 12110-2455. Each request must be accompanied by a self-addressed, stamped envelope. The list will be available for six (6) months, beginning on or about July 2, 2008.
25. DISQUALIFICATION: NYSUT Member Benefits Trust reserves the right, at its sole discretion, to disqualify from this or any Drawings it sponsors, now or in the future, any individual it finds to be tampering with the entry process or the operation of this Drawing or the Web site; to be acting in violation of these Rules; to be acting in violation of the terms of use of the Web site by cheating, hacking, deception or other unfair playing practices (including the use of automated entry programs), or to be acting in a non-sportsmanlike or disruptive manner, or with intent to annoy, abuse, threaten or harass NYSUT Member Benefits Trust or any person.
26. NYSUT Member Benefits Trust is not responsible for: (1) late, delayed, incomplete, destroyed, garbled, or incomplete entries, (2) incorrect or inaccurate transcription of entry information; (3) any human error, technical malfunctions, or lost-delayed data transmission; (4) any omission, interruption, deletion, defect, or line failures of any telephone network, computer equipment, or software; or (5) inability to access any Web site or online service, or any other error or malfunction.
27. NYSUT Member Benefits Trust reserves the right to modify, suspend or terminate the Drawing in the event it becomes infected by a computer virus or is otherwise technically impaired, and to cancel or suspend the Drawing in its entirety should tampering, unauthorized intervention, fraud, technical failures or other causes beyond NYSUT Member Benefits Trust’s control corrupt the administration, security, fairness, integrity or proper play of the Drawing. If this is the case, winners will be determined from among all eligible entries received prior to cancellation.
28. In the event of a dispute regarding the identity of the person submitting an online entry, the entry will be deemed to be submitted by the “Authorized Account Holder” of the e-mail address submitted at time of entry. Authorized Account Holder means the natural person who is assigned to an e-mail address by an Internet access provider, online service provider, or other organization that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address.
CAUTION: ANY ATTEMPT BY AN ENTRANT OR ANY OTHER PERSON TO DELIBERATELY DAMAGE THE WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS DRAWING MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, NYSUT MEMBER BENEFITS TRUST RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGES (INCLUDING ATTORNEYS' FEES) FROM ANY SUCH ENTRANT OR PERSON TO THE FULLEST EXTENT OF THE LAW, INCLUDING SEEKING CRIMINAL PROSECUTION.
