MAP - Member Assistance Program

In efforts to expand communications with members, NYSUT Member Benefits offers MAP, the Member Assistance Program. Just as maps provide direction, MAP team members will assist Member Benefits and vice versa. Through this e-mail-based program:

·  You’ll be among the first to hear about new, endorsed programs and program updates.
·  You’ll receive special offers.
·  We’ll ask for your opinions on new programs under consideration.
·  When possible, we'll ask you to help test potential new programs and give us your feedback.

Your opinions matter! You can help us shape future programs and services exclusively available to you and your colleagues. Help make a difference!

MAP e-mails are immediately recognizable by the subject line: MAP Alert. MAP e-mails are sent from Member Benefits approximately once every three weeks. We will not share your e-mail address with outside interests. We oversee the frequency of e-mail communications – You have our word we will not bombard you with e-mails!

MAP Drawing Rules


Eligibility

  1. Only NYSUT members and agency fee payers who enroll in MAP, NYSUT Member Benefits' Member Assistance Program, are eligible.
  2. VOID IN PUERTO RICO, THE VIRGIN ISLANDS AND WHERE PROHIBITED.
  3. Offer limited to legal residents of the United States who are 18 years or older.
  4. The following persons are not eligible to enter the Drawing: (a) employees, agents, consultants, officers and members of the Board of Trustees of NYSUT Member Benefits Trust ("Member Benefits"); (b) employees, agents, consultants, officers and members of the Board of Directors of NYSUT Member Benefits Corporation ("Member Benefits"); (c) employees, officers and members of the Board of Directors of NYSUT; (d) employees and officers of any organizations directly involved with this promotion; and (e) immediate family members and persons living in the same household as the persons identified in 4(a) through 4(d) above.
  5. Any NYSUT member or agency fee payer is eligible to win only once in any 12-month period.

Entry

1.       NO PURCHASE OR PAYMENT OF ANY KIND NECESSARY TO ENTER OR WIN THIS DRAWING (the “Drawing”).

2.       NYSUT members need not be present to win.

3.       NYSUT members are not required to: (a) purchase goods or services, (b) pay any money, or (c) submit to a sales promotion, to be eligible for the Drawing.

4.       NYSUT members may enter to win by enrolling in MAP at www.memberbenefits.nysut.org (the “Web site”).

5.       Limit one (1) entry per member or agency fee payer.

6.       NYSUT Member Benefits is not responsible for entries that are corrupted or otherwise not received correctly.

7.       NYSUT Member Benefits shall have no liability whatsoever for lost, late or misdirected entries.

8.   All entries become the exclusive property of NYSUT Member Benefits and will not be returned.

9.   Odds of winning depend on number of eligible entries received.

10.   Except as specifically set forth herein, all information provided by entrants will be subject to the NYSUT Member Benefits Trust Privacy Policy. The Privacy Policy may be viewed on the NYSUT Member Benefits Web site: or click here, or may be obtained by writing to: NYSUT Member Benefits , 800 Troy-Schenectady Road, Latham, New York 12110-2455. Such Privacy Policy is subject to change at any time, without notice, by NYSUT Member Benefits.

11.   By entering, each participant agrees: (a) to be bound by these Rules and the decisions of NYSUT Member Benefits, which are final and binding in all respects; and (b) to release and hold harmless NYSUT Member Benefits and its officers, employees, agents, trustees and directors from any and all liability for injuries, death, loss, tax liability, or damage of any kind arising from the participant's participation in this drawing or the acceptance, use or misuse of the prize.

12.   The winner(s) will be notified by e-mail or other means if it seems e-mail has not been received by the winner following conclusion of the drawing.

13.   The winner's name(s) will be posted on the NYSUT Member Benefits Web site following confirmation.

14.   Acceptance of a prize signifies winner’s consent that NYSUT Member Benefits or its agents may, without further consideration or restriction, use and/or publish the winner's proper name, city, state, likeness and prize information, for promotional purposes, whether online or in print or in any other media in connection with the Drawing, where legal. (Tennessee residents need not consent to, or sign, any publicity release.)

15.   The prize is not transferable by the entrant and may be subject to certain restrictions. No cash redemption or prize substitution is permitted except at NYSUT Member Benefits' sole discretion, and then only for a prize of equal or greater value.

16.   All taxes including without limitation, state, federal, and local sales, and income taxes, luxury taxes, and other expenses or costs associated with obtaining and using the prize are solely the responsibility of each winner. The winner agrees to indemnify, defend and hold harmless NYSUT Member Benefits from and against any claims, damages or penalties of any taxing authority or other government agency or third party in respect of such taxes or expenses.

17.   Prize, prize information, affidavits and statements of eligibility, if any, will be shipped to the address verified by the winner.

General Conditions

18.   This Drawing is sponsored and promoted by NYSUT Member Benefits, 800 Troy-Schenectady Road, Latham, New York 12110.

19.   The Drawing is governed by the laws of the state of New York, and all claims relating to the Drawing must be brought in the state or federal courts located in Albany County.

20.   Requests for the name of the winner may be sent to the following address: NYSUT Member Benefits, ATTN: Patrick Regan, 800 Troy-Schenectady Road, Latham, New York 12110-2455. Each request must be accompanied by a self-addressed, stamped envelope.

21.   NYSUT Member Benefits is not responsible for: (1) late, delayed, incomplete, destroyed, garbled, or incomplete entries, (2) incorrect or inaccurate transcription of entry information; (3) any human error, technical malfunctions, or lost-delayed data transmission; (4) any omission, interruption, deletion, defect, or line failures of any telephone network, computer equipment, or software; or (5) inability to access any Web site or online service, or any other error or malfunction.

22.   NYSUT Member Benefits reserves the right to modify, suspend or terminate the Drawing in the event it becomes infected by a computer virus or is otherwise technically impaired, and to cancel or suspend the Drawing in its entirety should tampering, unauthorized intervention, fraud, technical failures or other causes beyond NYSUT Member Benefits' control corrupt the administration, security, fairness, integrity or proper play of the Drawing. If this is the case, NYSUT Member Benefits reserves the right to offer the drawing again at a future date. 

23.   In the event of a dispute regarding the identity of the person submitting an online entry, the entry will be deemed to be submitted by the “Authorized Account Holder” of the e-mail address submitted at time of entry.  Authorized Account Holder means the natural person who is assigned to an e-mail address by an Internet access provider, online service provider, or other organization that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address.

CAUTION: ANY ATTEMPT BY AN ENTRANT OR ANY OTHER PERSON TO DELIBERATELY UNDERMINE THE LEGITIMATE OPERATION OF THIS DRAWING MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, NYSUT MEMBER BENEFITS RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGES (INCLUDING ATTORNEYS' FEES) FROM ANY SUCH ENTRANT OR PERSON TO THE FULLEST EXTENT OF THE LAW, INCLUDING SEEKING CRIMINAL PROSECUTION.
