New address for catastrophe plan claims
Marsh Affinity Group Services is in the process of moving the claims operation for the NYSUT Member Benefits Trust-endorsed voluntary Catastrophe Major Medical Insurance Plan from Park Ridge, Illinois to Des Moines, Iowa.
The claim migration began last March and will be completed in February 2009. Marsh hired additional staff to process claims. The goal of the move is to improve the percentage of first-call resolution and overall processing levels.
Claims already submitted to the Park Ridge facility are in the migration process. However, claimants should begin using this new address immediately for any new and existing CMM claims:

This notice pertains to the voluntary Catastrophe Major Medical Plan administered by Marsh Affinity. It does not apply to Group Catastrophe Major Medical Plans provided by locals, benefit funds or through contract negotiations with employers, which have a different administrator.
