EPIC seeks financial data from some seniors
EPIC, the state's prescription drug program for seniors, is notifying certain retiree members that it needs to obtain additional financial information from them.
EPIC is the Elderly Pharmaceutical Insurance Coverage program. EPIC subscribers who are single and earn $15,315 or less a year, and participating married couples who earn $20,535 or less a year, are being asked to provide some information about their assets and bank accounts. Eligible subscribers who meet these income guidelines may have already received a questionnaire from EPIC. They should complete and return the questionnaire as quickly as possible to the address indicated on the form.
EPIC will use this information to apply to Medicare Part D on members' behalf for extra help with paying their EPIC premiums.
In July 2006, EPIC was named the authorized representative for applying to Medicare Part D for EPIC subscribers.
Filling out the questionnaire does not affect a member's eligibility for EPIC.
Union members with questions about the questionnaire can call NYSUT Social Services at (800) 342-9810. UFT retirees can call UFT Retiree Social Services at (212) 598-6880.
